Every 3 months or so I teach a class that prepares my students for the paperwork involved in working in various massage settings. To do so they roleplay being clients and therapists for each other and in doing so they fill out simulated medical files as if they were really checking in to a medical center, chiropractors office etc.

Sometimes they fill these forms out as if for their real life selves and sometimes they make up characters and fill in the forms with fake or humorous things. Here are a few examples, with excerpts from the forms from my latest class:
